Avidity Home Health, a mission-oriented non-profit home health agency in sunny Southwest Florida, is seeking an experienced and team-oriented Physical Therapist to work in the home health setting.

The PT creates and maintains a plan of care, serving as the coordinator for patient and family interaction with Avidity for the duration of the patient’s treatment. The PT must be able to listen, observe, communicate and sensitively provide therapy services in emotionally intense situations.

The PT reviews and updates the care plan, physician orders, pharmacy profile and other records. The therapist also educates patients, family members and other caregivers on level of safety and functioning, to include patient’s ability to perform functional mobility and transfers.

Applicants must be a graduate of an approved therapy program and possess a current Florida PT license. Bachelor’s degree required. Minimum one-year experience in a home health environment providing direct patient care preferred.
