Job Description
ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provides individual, group, and family therapy to the clients and their families.
• Documents clinical information and billing according to Medicaid and agency standards.
• Knowledge in bio-psychosocial and In-Depth Clinical Assessment, CFARS, FARS, upon assignment of new admissions.
• Completes discharge summary at discharge.
• Meets productivity standards while maintaining established standard of care.
• Administers the Children’s Functional Assessment Rating Scale on all children in assigned caseload.
• Provides oversight for behavioral health programming associated with the treatment plan for all clients in assigned caseload
• Arranges home visits for the clients.
• Follow-up calls and meetings with other professionals or staff to discuss patient care and treatment may be necessary.
• Coordinates community resources as needed by individual clients.
• Facilitates treatment plan review/changeover meetings.
• Is responsive when there is a need for crisis counseling.
• Participates in the performance and quality improvement process.
• Ability to set appropriate limits and boundaries with clients, families, and staff members.
• Ability to handle confidential information appropriately in accordance with the provisions of federal and state laws.
• Visit patients in their homes for family counseling.
• Available to visit the court when required or when it’s beneficial to the client.
• To be available to respond clients emergency situations.
• Perform any other function assigned to it for the benefit of clients.
