Position Summary
Memory Care Program Coordinator manages the day to day activity operations of the Memory Care Community to ensure the maximum levels of resident, family and staff safety, wellness and satisfaction is met. Oversees the day to day operations of The Love is Ageless program to ensure the maximum levels of resident leisure contentment, as well as physical, mental and psychological well-being. This is accomplished by taking into account American House's Mission, philosophy, policies and procedures as well as current federal, state and local regulatory standards.

Qualifications and Required Experience for Memory Care Coordinator:
  • Related degree or experience in memory care or dementia care.
  • Minimum of 2 years of leadership experience, preferred.
  • Experience working with persons with dementia and related memory disorders.
  • Previous experience working with program implementation.
  • Computer skills.
  • Ability to be flexible with work schedule.
  • Actively express support for residents, staff, family members and guests.
  • Previous experience motivating others in fun and creative activities.
  • Strong organizational and time-management skills.
  • Problem solving and logic skills.
  • Interpersonal savvy.
  • Possess written and verbal skills for effective communication.
  • Teamwork.

Primary Responsibilities for Memory Care Coordinator:
  • Creates or oversee a diverse and therapeutic program calendar of events 7 days/week.
  • Coaches all staff members; to create and maintain a person-centric care philosophy of living in all that occurs in the Freedom Place neighborhood.
  • Continually evaluates effectiveness of program and demonstrates ability to implement changes as necessary utilizing the provided Memory Care playbook guide and score card for goals setting.
  • Conducts orientation for new team members ensuring all new employees are knowledgeable of the goals, policies, and procedures.
  • Oversees continuing education programs for both Memory Care team members as well as family members.
  • Acts as a Memory Care manager and resource person for the residents and their families.
  • Facilitates or ensures quarterly family support groups are held at Memory Care neighborhood or virtually when appropriate.
  • Oversees My Story Wall and Resident Recipe Card program.
  • Consistently displays a genuine care for others and driving a culture of caring within the Memory Care neighborhood.
  • Performs other duties consistent with the position as assigned.
