Lake City, FL 32024 Under the general direction of the Regional Director, the Healthcare Representative/admission nurse will educate medical referral sources about Community Hospice of Northeast Florida’s services, access to hospice care and delivery of end of life care with a strong emphasis on customer service. They will be well versed in appropriateness of services, and will be able to educate on a one to one basis as well as large group settings. Facilitates speed to care by providing a range of preadmission and admission services. Collaborates with internal and community partners. Provides education regarding Community Hospice services and philosophy. Collaborates with the manager, physicians, nurses and case managers to ensure the best outcome for patient, families and referral sources. Contributes to the smooth function of services, advocating for patient and family needs, coordinates care and facilitates transfer of care to appropriate team. Has an understanding of Medicare Conditions of Participation for Hospice, how insurances pay for Hospice and State Regulations that apply to Hospice admission. Provides education and information to referral sources, physicians, patients, and their families. Represents the organization in a positive professional manner understanding the importance of the first impression.
1. Achieve quarterly and annual referral and admission goals within assigned territory as set by the Regional Director.
2. Educate and motivate medical community about the benefits of hospice care and the unique benefits of Community Hospice.
6. Back-up fellow Healthcare Representatives as needed for special events or vacation time.
7. Maintain memberships in related health care professional organizations for the purpose of networking.
8. Communicate issues that arise in the field with clinical staff through defined channels to enhance service delivery.
9. Participate in organizational committees as identified that enhance the working relationships between departments within Community Hospice.
10. Represent Community Hospice professionally at all times through care and/or services provided to all clients.
11. Comply with Community Hospice policies, procedures and standard practices.
12. Maintain the confidentiality of patients and families within Community Hospice as per government mandates.
13. Use resources in a fiscally responsible manner.
14. Promote Community Hospice through participation in community and professional organizations.
15. Improve own professional knowledge and skill level.
16. Ability to implement Medicare Local Coverage Determination to assess patients for Hospice eligibility. Collects and communicates appropriate information to/from admission physician to determine hospice eligibility.
17. Collects and communicates appropriate information from referring and consulting physicians to help determine hospice eligibility. Performs an initial physical assessment, identifies immediate needs, to include spiritual, functional, psychosocial and financial needs.
18. Applies appropriate nursing interventions and documents concisely in electronic medical record. Facilitates admission to Community Hospice.
19. Provides emotional support to patients and families/caregivers with compassion and empathy.
20. Maintains the confidentiality of patients and families in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Assess for patient risk and safety issues per Community Hospice policies.
21. Develops patient’s initial plan of care based on identified immediate needs. Obtains and communicates physician orders for medications, DME and special treatments to the care team and in the electronic medical record.
22. Demonstrates knowledge of medications appropriate for end of life care, side effects and adverse interactions. Assess, communicate and obtain orders from Community Hospice physicians to ensure patient comfort.
23. Arranges, coordinates, and supports needed change in level of patient care.
24. Facilitates transfers from one level of care to the next based on Medicare Conditions of Participation. Complies with appropriate professional code of ethics in delivering services to patients and families
25. Professional development of self towards resource for end of life care, with knowledge, understanding and ability to communicate Medicare Conditions of Participation, Local Coverage Determination, State of Florida Regulations and specific insurance benefits.
26. Provides patients and/or family, Admissions Staff, and Community Partners consultation and information regarding Health Care Surrogate, Health Care Proxy and other Advance Directives.
27. Responsible for education and mentoring of new staff members.
28. Builds, develops and maintains relationships with referring physicians, area social workers, discharge planners and community agency staff in order to network and facilitate admission and placement of patients.
29. Educates medical community on what are hospice appropriate referrals

2. Educate and motivate medical community about the benefits of hospice care and the unique benefits of Community Hospice.
- Educate medical community on what are hospice appropriate referrals.

- Cultivate, develop and enhance existing relationships with physicians, hospitals, nursing homes and assisted living facilities.
- Expectation is that 8 to 10 physician calls will be made per day as well as numerous visits with office and hospital staff.
- “A” and “B” doctors will be seen at least two times per month.
- Identify potential high referral doctors within territory and document plan to increase referrals.
- Leverage in-service events, formal and informal education events to insure delivery of Community Hospice messages.
- Maintain accurate record of all expenses and report to Community Hospice on a timely basis.
- Validate all reported referral and admissions are reflected properly within Community Hospice records weekly and finalized within 30 days of the end of the month.

Required Licenses/Certifications:
- Current State of Florida driver’s license with driving record acceptable to Community Hospice insurance carrier, and reliable transportation.
Qualifications:
Education/Experience:
Any combination of education and experience that would provide required skills
and knowledge for successful performance would be qualifying.
Typical qualifications would be equivalent to:
- Bachelor’s degree or equivalent required.
- Minimum two years of documented sales success, preferably in selling services to medical professionals within the Jacksonville area.
- Clinical background.
- Fluent English, secondary language a benefit.

- Ability to communicate and articulate Community Hospice services and benefits to medical professionals in a clear and concise manner.
- Excellent presentation skills for both small and large audiences.
