Job Description:
Title: Registered Nurse
Employment Type: Per Diem, Full Time, Part Time

Requirements:

• Home Healthcare Experience for a Medicare Certified Agency
• Strong Assessment Skills
• Experience with OASIS charting and documentation
• Experience coordinating with other disciplines (LPN's therapy, HHA's,)
• Strong Clinical Skills
• Florida Registered Nurses License in good standing
• CPR Certification
• Current credentials and license in good standing
• Reliable Transportation (if auto - also Valid insurance and driver’s license)
• Valid Level II Background Check

Position Overview:

• Perform home visits assessments and procedures utilizing your extensive nursing experience and critical judgment skills
• Complete initial and ongoing assessments nursing care of all in home clients.

• Participate in ongoing interdisciplinary assessment of patients.

• Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s)

• Provide services that are ordered by the physician as indicated in the plan of care.

• Counsel and educate the patient/client and family in meeting healthcare related needs.

• Complete timely and accurate clinical notes including addressing the patient’s/client’s progress.

• Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.

• Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.

• Supervise paraprofessionals and licensed professional team members.

• Participate in: Quality Assessment and Performance Improvement (QAPI) program; Office Emergency Preparedness program
