We are a Medicare Certified Home Health Agency looking for RN Case Manager with home health experience to provide care for our patients. With offices in Stuart, Boca Raton, and Plantation, we serve patients in Broward, Palm Beach, Martin, St. Lucie, and Indian River Counties.

The RN Case Manager is responsible for the overall interdisciplinary plan of care for a patient from admission to discharge in a home care environment. He/she furnishes services in accordance with agency policy and procedures and acts as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectively.

The ideal candidate has a valid RN license, a complete understanding of case management, current certification in CPR, and experience working in home health. In addition, candidates must have a valid driver's license and current automobile insurance and must provide proof of the required health/background checks and training.

Responsibilities

• Provides case coordination for new clients with additional therapies (i.e., PT, OT, ST, MSW)
• Assumes on-call responsibilities during the week and weekends to alternate with other nurses
• Reviews the initial assessment of the patient and caregiver for appropriateness of and acceptance for home health services and to ensure it accurately reflects the patient's condition & needs
• Reviews the OASIS integrated assessment at requested time-points when nursing services are included in the plan of care or when requested by the agency
• Ensures that required documentation is complete in a timely manner and is in compliance with regulations and standards
• Reviews the Plan of Care in collaboration with the physician, all the professional disciplines involved in the case, the patient, and his/her caregiver
• Communicates appropriate preventive and rehabilitative nursing procedures to field clinical staff

Qualifications

• COMPASSION and a DESIRE TO HELP the elderly, the sick, and their families
• Valid RN license
• Experience in home healthcare required
• Unrestricted driver's license and clean driving record
• Reliable transportation and valid automobile insurance
• Current CPR card
• Certificate from an Alzheimer’s 2-hour course
• Level 2 background check required
• Health statement from the last 6 months
• Strong assessment and care planning skills
• Ability to delegate care appropriately
