Essential Duties and Responsibilities: (Note: Other Duties May Be Assigned) Timely review and completion of intake field reports from clinicians with a primary focus on other Physical Therapist and then transcribing and documentation in Oasis platform. Meet daily, weekly, and monthly quota of chart completions. Understanding of Agency policies and procedures and regulatory requirements. Current knowledge of rules and regulations relevant to OASIS documentation. Analyzes and evaluates clinical records to ensure agency policies and procedures are adhered to. Qualifications/Requirements: Graduate of an Accredited School of Physical Therapy. OASIS documentation, ICD-10 coding, QA, Compliance experience in Medicare Home Health: Plan of Care, Start of Care / SOC, Resumption of Care / ROC, Recertification, Discharge, HIS, preferred. OASIS (HCS-O, COS-C, or COQS), preferred. Formal clinical background required, PT, with current state license. Valid driver?s license and auto insurance. Proficient in the use of electronic documentation and EMR use. Must complete required departmental competencies to validate and maintain competency. Self-motivated who can work with little supervision. Education and/or Experience: Three to five (3-5) years of health care experience preferred. One (1) year of home health care experience required. Language Skills: Must possess good communication skills, both written and verbal in English. Ability to communicate in other foreign languages is desirable. Mathematical Skills: Math skills required Reasoning Ability: Able to work under pressure and handle multi-task functions. Must be able to deal effectively with people. Must be self-motivated and resourceful. Certificates, Licenses, Registrations: Licensed as a PT in the State of Florida. OASIS (HCS-O or COQS or COS-C) certification within the first 12 months of hire. Work Environment The work environment characteristics here are representative of those an employee encounters while performing the essential functions of the job. Once employee has successfully cleared orientation period, which include meeting productivity and accuracy goals, they may transition to a remote position. Remote position will require meeting daily, weekly and monthly productivity goals and accuracy rates.
