We are looking for qualified LPNs to provide the highest quality of care for our home health patients in a professional and compassionate manner. Field LPNs will use assessment tools to manage the medical needs of a patient and identify other disciplines needed to maximize the patient's outcome. Licensed Practical Nurses will be a patient advocate and will be in communication with the primary MD and Clinical Team Manager, as the patient's condition requires.
