We are now hiring Per Diem Registered Nurses for Medicare Home Health visits and OASIS assessments in PALM BEACH COUNTY. Medicare OASIS experience and one year RN home health experience mandatory! Registered Nurse (RN) Requirements; · Current state license as a Registered Nurse · Minimum 2 years nursing experience · Kinnser EMR system experience preferred · Current PPD or Chest Xray Registered Nurse (RN) Responsibilities: · Complete initial assessment (OASIS) · Initiate and develop plan of care · Provide health care instructions to the patient · Identify discharge planning needs as part of the care plan development and implements prior to discharge of the patient. · Update & communicate with the PCP / CM / ALF when necessary or condition changes.
